Clover Health will provide practices, at no cost, our point of care technology, The Clover Assistant. The Clover Assistant is powered by comprehensive Clinical and Claims data on Medicare beneficiaries. Using the Clover Assistant at the point of care will improve continuity of care and quality outcomes for your Medicare FFS patients, while significantly increasing your original Medicare FFS reimbursement.

CHP Participant Providers will see approximately a 40% increase in Provider Medicare E&M reimbursement, when submitting a qualifying Clover Assistant visit, and will be eligible for the Shared Savings Program.
A Focus on FFS Beneficiaries with Complex Chronic Conditions
CHP Participant Providers will collaborate with a network of specialists and ancillary facilities to improve care management, quality, and cost of care of FFS beneficiaries with complex chronic conditions.
